Right - to take up from a previous thread, I have constructed a couple of variations of a form Micky started. They're only simple, and in Word. The idea here is that we agree a design, I fill them all out for those attending FCS, print them in colour and pass them to Dave for laminating.

The logo at the top, one of you photochop people would probably be better off creating something.. but it's a start..

On the first one, the fields have been spaced symetrically. Font used is the forum font. It is on Alex's ftp site here:

The second one is the same style, but I've moved fields around to make them fit nicer. That one is here:

Could you let me know your feedback ASAP so I can get a final version hopefully agreed by the end of the week? If we end up with a couple of variations, I'll do a poll.

OH - and print or print preview the forms, the boxes do not appear when printing, but make it a damn sight easier to work on!!
